Diagnostic Considerations

For localized forms, consider seborrheic dermatitis and cutaneous lupus erythematosus. For generalized forms, consider pemphigus vulgaris. Also evaluate for other causes of erythroderma (eg, psoriasis, atopic dermatitis, cutaneous lymphomas, drug eruptions).

Endemic pemphigus foliaceus is treated mainly in Brazil with systemic corticosteroids, use of which is linked with osteoporosis, atherosclerosis, higher blood pressure, insulin resistance, glucose intolerance, hyperlipidemia, and abdominal obesity, adverse effects constituting criteria for the diagnosis of metabolic syndrome.
